This Seminar is designed to fulfill licensing authority or credentialing requirements for education to address recordkeeping concerns. It is also suitable for healthcare professionals who simply wish to improve their skills in this critical area.

It is applicable to physicians, physician assistants, advanced practice nurses and podiatrists. Other healthcare professionals, such as dentists, may also benefit from the principles and content presented in the seminar, and select sessions include dental-specific breakout discussions

There are many situations in which fresh information on medical recordkeeping can make a difference, including:

• Late or missing documentation
• Diagnosis and treatment are not supported by documentation
• Lack of explanation of clinical thinking
• Discrepancies between sections of the notes
• Poor documentation of prescribing practices

The Medical Recordkeeping Seminar is a one-day Seminar with an optional six-month Personalized Implementation Program. Both are offered remotely and in-depth engagement is expected from all participants.

Step 1: Enrollment - After your initial registration, you will receive a confirmation of enrollment within 48 – 72 hours.

Step 2: Preparation - At least three weeks prior to the seminar, you will have the option to submit up to three charts for review by a faculty member at no extra cost. In addition, you will receive an optional survey and self-audit.

Step 3: Online Seminar - The one-day Seminar runs from 8:00 am to 5:00 pm with a 30 minute lunch break. In the afternoon, there are small-group breakouts, including a Dentistry-Specific breakout group.

Step 4: Optional Personalized Implementation Program (PIP) - Upon completion of the one-day Seminar, participants have the option of enrolling in a six-month follow-up program that includes in-depth chart reviews, detailed feedback, and coaching to fully integrate documentation skills into participants’ daily practice.

Participants submit three sets of charts to CPEP at two, four and six months after enrollment. A physician reviewer provides detailed written feedback. Verbal feedback is available upon request. Participants also receive feedback and concrete suggestions for improvement.

At the end of the six-month program, participants receive a final report and grade of either Pass or Fail. The report can be released to a third party if requested by the participant.

• Are clear, concise and complete
• Accurately reflect the care provided
• Convey the rationale for the diagnosis or treatment plan
